Care Planning and Coordination in Home Health Nursing

The Health Promotion Seminar Series is a collaborative effort between de Souza Institute and the Community Health Nurses Initiatives Group (CHNIG). The seminars offer an opportunity for professional development to nurses working in all domains of practice; community or acute care. Topics are focused on current issues, and provide a means for knowledge transfer, discussion, and resources to apply to practice.
The monthly seminars are free of charge and open to all nurses. They are typically held on the third Thursday of the month from 12-1pm EST. Past presentations were very successful and covered the topics of Smoking Cessation, the Aging at Home Strategy, Chronic Disease Prevention, Cultural Safety, Primary Health Care, Political Action, Youth Engagement and the Social Determinants of Health. The next seminar is: Care Planning and Coordination in Home Health Nursing on June 21, 2012 with Karen Thompson (BScN, MEd., CCHN(C)). Karen is a Clinical Consultant working in the Professional Practice Department at ParaMed Home Health. She provides consultation to nursing and home support inquiries arising from any centre, supporting evidence-based policy and procedure development. Through the use of ParaMed’s Professional Practice Model she provides leadership and contributes to the growth and development of quality clinical service delivery. She is involved with designing, developing and implementing several clinical programs and enjoys the diversity of the work and participating in many activities that supports ParaMed’s innovative health care services. Prior to joining the ParaMed team Karen had experience as a staff RN in acute care hospitals and home care agencies, plus was a nurse educator with colleges and universities for 22 years. Karen is the co-leader for the Professional Practice team for the Community Health Nurses Initiative Group (CHNIG) of the Registered Nurses Association of Ontario.

Objectives for this seminar are to:
- Discuss how home health aligns with the health care service delivery team.
- Provide a brief overview of the Community Health Nursing Standards and how the role of the Home Health Nurse has evolved as a specialist.
- Discuss the realities and challenges in home health care by sharing a day in the life of home health nurse.
