VHA Home HealthCare understands that partnership is essential to driving
positive change in complex systems. We value our partners. To help
ensure the best possible working relationships, and effective and
successful partnerships, we are conducting our Annual Partners’ Feedback
Survey. The survey findings will identify where VHA can improve and help
pave the way for long-lasting, positive partnerships in future.

As a key community partner, we hope you will complete the survey to
provide feedback on your experience working with VHA.  The survey is
confidential and anonymous and should take only 5-10 minutes to
complete. A summary of the key findings will be shared when these are
available.
